Important Program Qualification Guidelines
  • Nurses and caregivers must be actively employed at the time of bonus payout.
  • Referrals must be new to PCM unless qualifying under Last Touch criteria.
  • The nurse or caregiver submitting the referral cannot conduct the worker's initial assessment.
  • Bonus determinations are made at the sole discretion of PCM management.
What qualifies for the $2,000 bonus?
A referral qualifies when the worker becomes a staffed PCM client and all program requirements have been met.
What if the worker doesn't have a Department of Labor White Card?
PCM can help determine eligibility and guide workers through the claims process when appropriate.
How long does the referral process take?
Every case is different. Some referrals may move quickly, while others require additional eligibility verification and claims support before services can begin.
